Short answer
The IRIS Taxpayer Portal's CSV templates (TY2025 guidelines) have 66 columns for 1099-INT (A–BN), 69 for 1099-DIV (A–BQ) and 67 for 1099-R (A–BO). 1099-INT adds the payer's routing number (exactly 9 digits starting 01–12 or 21–32); 1099-R has no 2nd TIN Notice column and requires the Box 7 distribution code. In every template, TINs keep their dashes, money is digits with up to two decimals and blank instead of 0, check boxes are Y or N, cells are formatted as Text, and each file holds one tax year.

What's different on each form
- 1099-INT adds the payer's RTN (routing number, 9 digits starting 01–12 or 21–32), a foreign country code in Box 7, and a CUSIP number (up to 9 letters and digits) in Box 14.
- 1099-DIV has the most boxes (1a through 13; box 11 is the FATCA check box, which sits near the start of the template), including the Section 897 boxes 2e and 2f and Section 199A dividends in Box 5.
- 1099-R has no 2nd TIN Notice column. Box 7 distribution code is required; boxes 2b and IRA/SEP/SIMPLE are Y/N; boxes 8 and 9a percentages are up to 5 characters; Box 11 is a four-digit year; Box 13 is a date in MM/DD/YYYY.
Rules shared by every IRIS template
- TINs keep their dashes: XX-XXXXXXX for EINs, XXX-XX-XXXX for SSNs, ITINs and ATINs.
- Name Type B fills the business name; I fills first and last name, which only allow letters, spaces and hyphens.
- Domestic ZIP codes are 5, 9 or 12 digits with no dash; states are two-letter codes; the country is an IRS country code (US for the United States).
- Money: digits and up to two decimals, no $ or commas, and blank instead of 0.
- Each state section needs its state code and at least one amount. CF/SF is a quoted list such as "AL,CA".
- Format cells as Text and save as CSV UTF-8, with one tax year per file.
These rules come from the IRS's TY2025 template formatting guidelines. Some example values in those guides still show older tax years, so download the current template from the IRIS Taxpayer Portal and check your header against it before you upload. Questions
IRIS template questions
How many columns are in the IRIS 1099-INT CSV template?
66 (A through BN): 36 payer and recipient columns, FATCA and 2nd TIN Notice check boxes, the payer's routing number (RTN), boxes 1 through 14, the CF/SF state list, and two state sections of six columns each. The 1099-DIV template has 69 columns (A–BQ) and 1099-R has 67 (A–BO).
What goes in the 1099-R distribution code column?
Box 7 is required: one or two characters. The first must be 1–9, A–H, J–N, P–U, W or Y, and a second code must be a valid combination with the first, as listed in the Instructions for Forms 1099-R and 5498.
Does the IRIS 1099-R template have a 2nd TIN Notice column?
No. The 1099-INT and 1099-DIV templates do, but the 1099-R template goes straight from FATCA Filing Requirements to Box 1.
How do I enter amounts in an IRIS CSV template?
Digits with at most two decimal places: no dollar signs, commas or negatives. Leave a box blank rather than entering 0 or $0.00, and format the column as Text so the spreadsheet doesn't turn long numbers into scientific notation.
What is the RTN field on the IRIS 1099-INT template?
The payer's routing transit number. It's optional on the form; if you enter it, it must be exactly 9 digits starting with 01–12 or 21–32. See the 1099-INT instructions for when to use it.
Are my CSV rows uploaded anywhere?
No. The checker reads the file in your browser; nothing is sent to EFileForMe or the IRS.
